(d)During the last five years, Michael Taglich has not been convicted of a criminal proceeding (excluding traffic violations or similar misdemeanors).

 

(e)During the last five years, Michael Taglich was not a party to a civil proceeding of a judicial or administrative body of competent jurisdiction.

Item 3.  Source or Amount of Funds or Other Consideration.

  

The source of funds is personal funds. The amount of funds invested in the beneficially-owned shares to date is $900,534.

The purpose of the transaction is solely for investment.

  

Item 5.  Interest in Securities of the Issuer.

  

Michael Taglich beneficially owns 1,908,258 shares of common stock of Intellinetics, Inc., having both voting power and dispositive power over such common stock. This represents 21.7084% of the common stock of Intellinetics. Inc. During the past 60 days, Michael Taglich purchased a convertible note (on March 11, 2015) with a principal balance of $50,000, convertible at a price of $0.30 per share into 166,667 shares of common stock. Such transaction was effected in cash directly with Intellinetics, Inc.
